Patient-Centered Medical Home is seeking a full or part-time Primary Care / Family Medicine Physician to join its team of providers at its location about 40 minutes northeast of Montpelier. The family practice is part of a Rural Health Center that schedules patients Monday Friday. There are no patients under the age of 6, with patients between the ages of 6-18 representing
